Scope Boundaries (Who/What it applies to)
This template applies to all Site Supervisors, Project Managers, General Contractors, and Subcontractors operating on commercial, industrial, and institutional (ICI) construction projects in Ontario. The scope of this daily reporting framework covers the contemporaneous tracking of active on-site labour forces, heavy equipment utilization, local weather conditions, authorized site visitors, documented schedule delays, and field instructions issued by the Prime Consultant or Owner. It serves as the primary administrative record to defend against delay claims, substantiate change orders, and verify health and safety compliance under Ontario jurisdiction.
Mandatory Deliverables & Administrative Windows
- Daily Submission Window: The completed and signed Daily Site Report must be uploaded to the project management information system (PMIS) by 08:00 EST of the following business day.
- Labour and Equipment Log: Detailed headcount by trade, subcontractor name, hours worked, and active versus idle heavy equipment tracking.
- Weather Impact Record: Ambient temperature, precipitation, and wind speed recorded at 07:00, 12:00, and 16:00, explicitly noting any weather-induced work stoppages or critical path impacts.
- Visitor and Safety Log: Registry of all non-working visitors, Ministry of Labour (MOL) inspectors, and daily safety talks or toolbox meetings conducted.
- Field Instructions & Delays: Immediate logging of verbal instructions, differing site conditions, or utility conflicts, cross-referenced with CCDC Contemplated Change Notices (CCNs).
Ontario Regulatory Compliance Notes
All site reporting must align with the *Occupational Health and Safety Act* (OHSA) and *O. Reg. 213/91 (Construction Projects)*, ensuring that daily inspections of work areas, excavation shoring, and scaffolding are logged.
Furthermore, because daily site records directly substantiate progress claims, schedule impacts, and potential disputes, this template incorporates the strict January 1, 2026 Ontario Construction Act amendments (Bill 216 and Bill 60):
- Proper Invoice Verification: Invoices are legally deemed proper unless the owner gives a formal written notice of deficiency within 7 days after receiving the invoice. Missing the 7-day notice window forfeits the right to dispute invoice form or structure and starts the 28-day prompt-payment timeline.
- Annual Holdback Release: For contracts extending beyond one year, owners must publish a Notice of Annual Release of Holdback (Form 6) on an electronic construction trade news website within 14 days after each contract anniversary. Payment of the accrued basic holdback must occur no earlier than day 60 and no later than day 74 after publication, provided no liens are preserved.
- Holdback Refusal Repeal: The former section 27.1 notice-of-non-payment mechanism for refusing holdback distribution due to deficient or incomplete work is repealed. Holdback use is restricted to formal contract abandonment or termination scenarios and preserved lien risk.
- Adjudication Timelines: Parties have a strict 90-day window after contract completion, abandonment, or termination to initiate interim binding adjudication through ODACC or a qualified private adjudicator.
Form/Sign-off Requirements
- Supervisor Signature: Mandatory digital or wet signature of the designated Site Supervisor certifying the accuracy of labour and safety logs.
- Project Manager Sign-off: Review and sign-off within 24 hours of submission to validate delay claims and potential change directives.
- Photographic Attachments: Minimum of four high-resolution, time-stamped, and geo-tagged photographs showing site conditions and progress.
- Change-Order Acknowledgement: Explicit sign-off field for any verbal instructions received from the Consultant, noting that work will not proceed without a written Change Directive or CCN.
- Record Retention: All daily reports, visitor logs, and safety records must be archived in the PMIS for a minimum of seven (7) years to comply with the Ontario Limitations Act.
