This template establishes the mandatory access control protocols for all personnel entering the physical boundaries of an active Ontario construction project. It applies directly to General Contractors and Site Supervisors executing their statutory duties as the "Constructor." The scope encompasses all site personnel, including subtrades, delivery drivers, consultants, municipal inspectors, and owner representatives. It governs physical entry points, verification of site-specific safety orientations, tracking of restricted-area access (such as confined spaces or high-voltage zones), and real-time muster accountability during emergency evacuations.

Mandatory Deliverables & Administrative Windows

To maintain compliance and project security, the following deliverables and administrative timelines must be enforced:

  • Pre-Access Orientation Verification: Completed and logged prior to any individual's first entry onto the project site.
  • Daily Sign-In/Sign-Out Log: Executed in real-time at the designated site access point; no retroactive logging is permitted.
  • Restricted-Area Access Permits: Issued and logged at least 2 hours prior to the commencement of specialized high-risk work.
  • Daily Log Reconciliation: Completed by the Site Supervisor within 1 hour of the end of each shift to verify all personnel have safely exited.
  • Subcontractor Crew Lists: Submitted by trade contractors to the Site Supervisor 24 hours in advance of scheduled site arrival.

Ontario Regulatory Compliance Notes

Under the Occupational Health and Safety Act (OHSA), the Constructor bears ultimate responsibility for health and safety coordination. This log serves as primary documentary evidence of active site management and due diligence.

Furthermore, site access controls and administrative logs directly impact contract administration, progress billing, and dispute resolution under the Ontario Construction Act. Under the January 1, 2026 amendments (Bill 216 and Bill 60), the following statutory controls apply to all project administration, payment, and closeout procedures:

  • Proper Invoice Disputes: Invoices submitted by trade contractors are legally deemed proper unless the owner delivers a formal written notice of deficiency within 7 days of receipt. Missing this 7-day window forfeits the right to dispute the invoice's form or structure and immediately triggers the 28-day prompt-payment timeline.
  • Annual Holdback Release: For contracts extending beyond one year, owners must publish a Notice of Annual Release of Holdback (Form 6) on an electronic construction trade news website within 14 days after each contract anniversary. Payment of the accrued basic holdback must occur no earlier than day 60 and no later than day 74 after publication, provided no liens have been preserved.
  • Repeal of Section 27.1: The former section 27.1 notice-of-non-payment mechanism for refusing holdback distribution due to deficient or incomplete work is repealed. Holdback use is strictly restricted to formal contract abandonment or termination scenarios and preserved lien risk.
  • Interim Adjudication: Parties have a strict 90-day window after contract completion, abandonment, or termination to initiate interim binding adjudication through ODACC or a qualified private adjudicator to resolve outstanding payment or access-related disputes.

Form/Sign-off Requirements

  • Daily Supervisor Sign-Off: The designated Site Supervisor must sign and date the log daily, certifying that all entries are complete and accurate.
  • Visitor Acknowledgement: Every non-regular visitor must initial the log, confirming they have received a visitor briefing and agree to abide by site safety rules.
  • Training Credentials Attachment: Copies of valid Working at Heights, WHMIS 2015, and trade-specific licences must be digitally attached to the user's access profile prior to log activation.
  • Record Retention: All completed access logs and signed safety acknowledgements must be retained in the project database for a minimum of 7 years following project completion.
